This is not a HA, but Blackpool Council's ALMO (Blackpool Coastal Housing) has recently established formal tenant scrutiny arrangements. This icludes an overarching Tenants and Leaseholders Assembly (which acts as a Committee), underneath which sits various Service Improvement Panels. These are all made-up of tenants and have various ways in which issues can be flagged up and reviewed using a scrutiny approach.
